Hours and Locations
Childhood immunizations (ages 0-18) are available in our walk-in clinics in Yulee and Hilliard Clinics. For more information about times and costs, call our clinical services at 904-875-6100.
Bring the following:
- Copy of the child's immunization record
- A parent must accompany the child. If another adult is bringing the child, they must present a notarized written consent from the parent or guardian. The consent must include:
- Current date
- Child's name
- Child's date of birth
- Parent's signature
- Parent's contact phone number
Programs and Services Offered (Ages 0-18)
Vaccines for Children Program: The program offers immunizations at no cost for eligible children through 18 years of age, including those without insurance or on Medicaid. Children with private insurance should see their doctor for immunizations.
Required Immunizations: DOH-Nassau provides immunizations required for childcare centers and schools. DOH Form 680 is available upon request. DOH Form 681 can be obtained through an appointment with one of our nurses and requires administrator approval. Visit our school health requirements for more infomation.
TB Tests for Work or School
Tuberculosis (TB) screening and skin testing are available at our Yulee, Fernandina, and Callahan Clinic sites. Be aware that skin tests are not performed on Thursdays.
Cost includes a flat fee for the TB skin test and a sliding fee scale for the visit. For current costs and scheduling, call any of our clinics at 904-875-6100.
