Primary Care

Florida Department of Health in Nassau County

The Florida Department of Health in Nassau County provides Primary Care services for residents of Nassau County. There are eligibility requirements for this program including financial, residency and diagnosis.

You must have one of the following qualified chronic conditions:
  • Diabetes

  • High Blood Pressure

  • High Cholesterol

  • Heart Disease

  • Asthma requiring daily treatment

  • Arthritis

  • Chronic Obstructive Pulmonary Disease (COPD)

  • Hypothyroidism

Note:  Our Primary Care Program does not provide services for individuals needing only chronic pain management or behavioral/mental health services and recommend if you need these types of services please seek care through a speciality clinic.
