WIC—Women, Infants, and Children

Florida Department of Health in Nassau County

WIC is a federally funded nutrition program for women who are pregnant or breastfeeding or who have recently been pregnant, infants & children under age 5.

WIC provides the following at no cost:

  • Healthy foods
  • Nutrition education and counseling
  • Breastfeeding support
  • Referrals to health care, immunizations, and community services

WIC EBT Card

EBT WIC Client LogIn

Florida WIC clients use a WIC EBT card to purchase WIC approved foods at authorized WIC grocery stores.

If you have a WIC EBT Card and have questions or problems using your card, call< WIC EBT Customer Service at 1-866-629-1095.

Check your balance before shopping by calling WIC EBT Customer Service at 1-866-629-1095 or by going to the WIC EBT website.

If there is a WIC EBT Point of Sale (POS) machine at the store, you can slide your card and enter your PIN to get your balance.


WIC Eligibility

  1. Income Eligibility
    • You automatically meet the income criteria if you participate in Florida Medicaid, Food Assistance Program, or TANF (Temporary Assistance for Needy Families).
    • If you are employed, the gross income level of your household (before taxes and deductions) must fall below 185% of the poverty level. Please check the WIC Income Guidelines to see if you qualify.
  2. Nutritional or Medical Risk
    • All participants in WIC will receive a health assessment to determine if they are at nutritional or medical risk.

Apply for WIC

Call 1-800-342-3556 to schedule your WIC Certification Appointment. You will need to bring the following to your Certification Appointment:

  1. Physical Presence
    • When required, bring each woman, infant, and child applying for WIC.
  2. Proof of Income
    • Proof of ALL income for everyone in the household before taxes and deductions are taken out.
    • Proof of income must include all sources of funds received during the last 28-31 days.
    • If you receive Medicaid, Temporary Cash Assistance (TCA), or food assistance (SNAP), your proof of income won’t be needed.
  3. Proof of Residency
    • One proof of where you live (no P.O. boxes).
    • You must be currently living in Florida, but you don't have to be a U.S. citizen.
  4. Proof of Identification
    • One proof of identification (ID) for you and one proof of ID for any infant or child applying for WIC.
  5. Completed WIC Medical Referral Form
  6. Social Security Number
    • Social Security Number for each person applying for WIC, if available.
  7. Immunization Records

Breastfeeding Warmline

DOH-Nassau WIC Program offers a breastfeeding warmline to all WIC clients. Talk to a Certified Lactation Counselor for answers to questions and problem solving assistance. The Warmline is available 7 days a week from 8 a.m. – 8 p.m. Please call 904-344-6317.
