A Community Health Improvement Plan (CHIP) is a long-term, systematic effort to address public health issues based on the results of community health assessments and the community health improvement process. The plan, which is updated every three to five years, involves a collaborative effort to identify and address health problems, develop measurable health objectives, and implement coordinated strategies. It is not limited to traditional public health categories but includes various community issues that impact health, such as environmental, economic, and housing concerns. Ultimately, CHIP aims to improve community health through enhanced coordination, knowledge, and partnerships.

The Florida Department of Health in Nassau County (DOH-Nassau) is seeking dedicated volunteers to join two vital workgroups of the Nassau Community Health Improvement Plan (CHIP).


Community Support Workgroup

The Community Support workgroup is developing an intergenerational program called Bridging the Years - Teens and Seniors Mixing It Up! This initiative brings Nassau seniors and teens together to share talents, resources, and mutual support. We need your innovative ideas and suggestions for implementation. We are also seeking coordinators to help organize this initiative.


Health Disparities Workgroup

The Health Disparities workgroup is dedicated to reducing health disparities across Nassau County. This group provides targeted health education to various community sectors, including faith-based organizations. We are also looking for volunteers to champion the Best Babies Zone community project, which focuses on reducing infant mortality by following the national model's framework. Volunteers passionate about improving the health of infants, women, and families in targeted areas of Amelia Island are encouraged to join.
