Nassau Health Improvement Coalition (NCHIC)
Vision: Improving the health of Nassau County through awareness, education, partnerships, prevention, service delivery, and policy development so that individuals can live healthy, active lifestyles.
Mission: Enhancing the health status of the Nassau County community by addressing and monitoring priority health and wellness issues through a collaborative, community-driven process.
About NCHIC: In early 2007, the Nassau County Health Advisory Council rebranded as the Nassau County Health Improvement Coalition (NCHIC), revising its mission and vision. NCHIC includes various local agencies, organizations, community groups, and members united by the goal of enhancing community health. The coalition meets bi-monthly on the fourth Monday at Yulee Full Service School (Nassau Family Education Center) – 86207 Felmor Road, Yulee, FL 32097.
Initiatives and Projects Supported by NCHIC
- Walk With Me: Challenges community members and organizations to create walking groups and participate in Walk With Me events.
- Partnership for a Healthier Nassau: A collaborative group that completed the MAPP process and developed the Community Health Improvement Plan (CHIP). The PHN Steering Committee monitors the plan implementation for the next three years. More information is available on our Community Health Planning page.
- Walk With a Doc: Dr. Kiersten Prince, DO, with Baptist Primary Care in Callahan, sponsors a monthly walk with patients from Ewing Park. The program starts with a 30-minute walk, aiming to gradually increase to 45 minutes.
- Nassau County School District Worksite Wellness Program: Implements best practices for worksite wellness with on-site exercise and nutrition classes, wellness libraries, and resources available for employees. Some activities are also open to the community.
- Health Education Lending Library: Sponsored by the Florida Department of Health Nassau County, this free resource equips agencies with tools and resources to host health education classes and programs.
