Clinic and Counseling
Individuals have access to pregnancy testing and counseling, physical examinations, screening for hypertension, breast and cervical cancer, and sexually transmitted diseases, including HIV counseling and testing. Referral services are offered as needed. Call 904-875-6100 for appointments.

Eligibility
We offer universal eligibility with an emphasis on low-income individuals, utilizing a sliding fee scale. Please provide current and consecutive income information for all employed household members. For additional eligibility questions, please contact one of our clinic locations.
