DOH-Nassau County offers Primary Care services for residents of Nassau County. To qualify for this program, certain eligibility requirements must be met, including financial criteria, residency, and diagnosis.
Our Primary Care Program does not provide services for:
- Individuals needing chronic pain management only
- Individuals requiring behavioral or mental health services
For these types of services, we recommend seeking care through a specialized clinic.
To be eligible for our Primary Care Program, you must have one of the following chronic conditions:
- Diabetes
- High Blood Pressure
- High Cholesterol
- Heart Disease
- Asthma requiring daily treatment
- Arthritis
- Hypothyroidism
- Chronic Obstructive Pulmonary Disease
