The Florida Department of Health in Nassau County offers Primary Care services for the residents of Nassau County. This program serves as the medical home for residents meeting specific financial, residency, and diagnosis requirements.
How to Qualify
1. Financial Qualification
- Must be at or below 150% of the poverty level.
- Proof of income is required.
- Download the verification of income form.
2. Residency Requirements
- Must be a Nassau County resident for at least 6 months.
- Must be a Florida resident for at least 1 year.
- Proof of residency is required.

3. Chronic Condition Requirements
You must have one of the following chronic conditions:
- Diabetes
- High Blood Pressure
- High Cholesterol
- Heart Disease
- Asthma requiring daily treatment
- Arthritis
- Chronic Obstructive Pulmonary Disease (COPD)
- Hypothyroidism
Important Note
Our Primary Care Program does not provide services for:
- Individuals needing only chronic pain management
- Individuals requiring behavioral or mental health services
